Tesamorelin is a synthetic 44-amino acid peptide analog of growth hormone-releasing hormone (GHRH). It is specifically modified with a trans-3-hexenoyl group attached to the N-terminal tyrosine residue, which increases its stability and potency compared to natural GHRH.

Tesamorelin acts selectively on the pituitary gland, stimulating the pulsatile release of endogenous growth hormone (GH). Unlike direct growth hormone injections, it preserves the body’s natural feedback mechanisms and maintains more physiological GH secretion patterns. This results in increased production of insulin-like growth factor-1 (IGF-1) primarily in the liver.

Originally developed and FDA-approved for the reduction of excess visceral (abdominal) fat in HIV-infected patients with lipodystrophy, Tesamorelin is valued for its ability to target fat reduction while largely sparing subcutaneous fat and muscle tissue. It is administered via daily subcutaneous injection.

Research has confirmed several important therapeutic effects of Tesamorelin:

• • Reduction of Visceral Adipose Tissue (VAT) Tesamorelin is highly effective at selectively decreasing visceral fat accumulation. In HIV patients with lipodystrophy, clinical trials showed significant reductions in visceral adipose tissue (15–20% on average) after 26 weeks of treatment, with improvements maintained during longer-term use. Importantly, it reduces waist circumference and VAT without causing substantial loss of subcutaneous fat or lean muscle mass.

• • Improvement in Body Composition By increasing endogenous GH and IGF-1 levels, Tesamorelin improves overall body composition. Studies demonstrate favorable changes including decreased trunk fat, reduced waist-to-hip ratio, and modest increases in lean body mass in certain populations.

• • Metabolic and Cardiovascular Benefits Reduction in visceral fat with Tesamorelin is associated with improvements in metabolic parameters. Some studies report better lipid profiles (including reduced triglycerides), decreased liver fat, and potential cardiovascular risk reduction linked to lower visceral adiposity. It may also improve insulin sensitivity in specific patient groups, although effects on glucose metabolism require careful monitoring.

• • Cognitive Function Emerging research suggests Tesamorelin may offer neuroprotective benefits. In older adults and individuals with mild cognitive impairment, treatment has been linked to improvements in executive function and verbal memory, likely mediated through increased GH and IGF-1 levels which support neuronal health and neuroplasticity.

• Other Potential Applications Due to its ability to elevate GH and IGF-1 in a more physiological manner, Tesamorelin is being investigated for age-related decline in growth hormone (somatopause), non-alcoholic fatty liver disease (NAFLD), and certain metabolic disorders. It shows promise as a safer alternative to direct GH therapy because it respects natural regulatory feedback loops.
